It’s important to have someone who can help you navigate through the healthcare system and provide support when you need it. Our case manager is a licensed registered nurse, who follow the national standards established by the Commission for Case Management.

Our Care Manager- Teresa- is ready to help get you back on your feet or get you on the path to managing your chronic conditions. Your care
manager will promote frequent communication and collaboration between you and your health care team to help you achieve your best possible health. Patients can get support with care coordination for care regarding chronic disease management.

    Camille Evans, LMSW

    Throughout her 16-year career in healthcare, Camille Evans has served in a variety of positions including roles in leadership, social work and as a behavioral health consultant. Camille is passionate about helping people in her community improve their overall health and quality of life. Having five children has taught Camille the challenges of parenting and the need to work together and learn from each other. Camille values every individual, and believes total health is a balance of physical, mental, emotional and spiritual elements.

      Madison Beatty, MD


      Dr. Beatty was drawn to Family Medicine because it allows her to build true relationships with patients of all ages and provide full-spectrum family care. An Idaho native, she is thrilled to practice in her home state and particularly passionate about rural health where she can be involved in her community and build strong relationships with both her patients and her neighbors.

      Dr. Beatty is fluent in both English and Spanish.

      Rachel Oliver, MD

      Dr. Oliver received her BA in Political Science with a Minor in Chemistry from the University of North Carolina, Chapel Hill, North Carolina in 2000. She completed her OB/GYN residency at Riverside Regional Medical Center in Newport News, Virginia in 2013. She has been serving the Treasure Valley since 2017 as active staff of OGA Women’s Health services. Dr. Oliver grew up the daughter of missionaries, living in Mexico as well as various parts of the United States. She became fluent in Spanish at a young age and chose to attend medical school in Mexico. She and her husband have two small boys and enjoy a variety of outdoor activities such as skiing and hiking.

      Patrick M. Plummer, MD: FAASM is an ENT-otolaryngologist.

      He received his medical degree from University of Kansas School of Medicine and finished his residency at Medical College of Wisconsin Affiliated Hospitals. He has been in practice for more than 20 years and he is a Diplomate of American Board of Sleep Medicine.

      Dr. Jerod Hines

      Jerod Hines, MD

      Dr. Jerod Hines was born and raised in Southeast, Idaho. He received his medical degree from University of Utah School of Medicine and completed his residency and received his Fellowship of Sports Medicine at McGill University, Department of Orthopedics. Dr. Hines served as a VISTA volunteer in Lowell, MA for 1 year and also served as Hospital Corpsman in Navy Reserves for 8 years.

      Dr. Hines has been living and working full-time in Gem County since 2016. He is the Director of Valor Health’s Sports Medicine program and you can usually find him on the sidelines of many Emmett Huskies sporting events. Dr. Hines enjoys spending time with his son, coaching and playing sports and spending time outdoors.
