Trever Martin, DPM

Trever Martin, DPM

Bio

Trever J. Martin, DPM is a graduate of Brigham Young University – Idaho and Temple University School of Podiatric Medicine.  He trained in foot and ankle surgery and wound care at North Colorado Medical Center (NCMC).

Dr. Martin provides compassionate care to his patients, specializing in foot and ankle surgery, general foot and ankle care, sports medicine, pediatrics, diabetic foot care, and wound care. He has additional training in pediatric foot and ankle conditions and surgery and has completed several medical missions to Mexico providing pediatric care.

Dr. Martin is a member of The American College of Foot and Ankle Surgeons and The American Podiatric Medical Association.  He is fluent in Spanish and Portuguese.

Dr. Martin enjoys spending time with his family, coaching and playing sports, and enjoying the Idaho outdoors.

Zack Bastian

Zachary Bastian, MD FACS, is a native of Boise, Idaho, who knew at an early age that he wanted to be a surgeon. After graduating from the University of Idaho, he studied medicine at Georgetown University in Washington, DC. During that time, he met his talented wife, Dr. Allison Bastian. Together, the two moved to North Dakota, where Dr. Bastian completed his General Surgery residency, emphasizing Rural Surgery. Finally, after more than 10 years away from home, Drs. Bastian returned to Idaho, along with their infant daughter, eager to start their new lives and careers.

Fast forward some years and Dr. Zachary Bastian has worked full-time in Emmett for the past 4 years and serves as the VH Director of Surgery. When not performing surgery, Dr. Bastian loves to spend time outdoors with his family, hiking and camping. He likes to work with his hands and is known as a craftsman, gardener, and handyman at home.

Sleep Medicine

Brian Fortuin, MD

Brian Fortuin, MD, is a University of California- Santa Barbara graduate. He attended medical school at Duke University and had an internal medicine residency at the University of Washington in Seattle. He received sleep medicine training from Stanford University and became board-certified in sleep in 1997.

Dr. Fortuin enjoys rural medicine, having a more direct impact on the people and community around him. He will offer an in-person clinic monthly at Valor Health Specialty Medicine and Surgical Clinic, working closely with our sleep lab technologists to diagnose and treat sleep disorders. When not providing care at Valor Health, he works as a general internist at his Twin Falls, Idaho medical practice.

When not providing care, Dr. Fortuin enjoys the great outdoors with his wife: alpine skiing, rafting, and golfing. He also enjoys aviation, flying in the backcountry, spending time on his small farm, and raising crops and sheep.

Pain Management

Chancen Hall, DO

Dr. Chancen Hall is a fellowship-trained interventional pain management specialist and anethesiologist.  He earned a B.S. in Biology with Honors from Dixie State University in St. George, Utah in 2013 and graduated with his Doctor of Osteopathic Medicine degree from Rocky Vista University in 2018. He subsequently completed an internship and anesthesia residency at the University of Kansas—Wichita, followed by a fellowship in chronic pain management at Penn State University in 2022. In 2023, Dr. Hall joined the Advanced Pain Management center of Oklahoma, providing comprehensive chronic pain care through advanced techniques such as spinal and joint injections, peripheral nerve stimulation, radiofrequency ablation, spinal cord stimulation, neurolytic procedures, and sympathetic blocks.

Excited to be back in a rural community, Dr. Hall is originally from a small town in southern Utah.  When he's not practicing medicine, he cherishes time with his wife and kids and serving his community.  He is always on the lookout for a new mountain bike trail or disc golf course.

Tyler Jue MSN, ACNP-BC, CWCN-AP, CWS

Tyler Jue is a board-certified acute care nurse practitioner, certified wound care nurse (advanced practice) and certified wound specialist. He has over a decade of experience in advanced skin and wound care. He holds two national wound certifications from the American Board of Wound Management and the Wound, Ostomy and Continence Nurses Society.

He takes pride in offering current advanced wound care, in addition to educating patients and empowering them with how to best prevent recurring wounds.

When not at work, Tyler enjoys spending time with his wife and children exploring Idaho’s great outdoors, including mountain biking, fly fishing and bow hunting.

Robert (Bob) L Rice, MD

Dr. Bob Rice is a dedicated physician who specializes in general medical hematology and oncology at Intermountain Health's St. George Cancer Center. He works with patients via telehealth and focuses on diagnosing, treating, and preventing various cancers, including lung, breast, prostate, and more.

Dr. Rice prioritizes understanding each patient's unique circumstances and aspirations while utilizing the latest medical research and clinical guidelines. His approach centers on personalized, evidence-based care, collaborating closely with patients to help them achieve their health goals and enhance their overall quality of life.

Dr. Rice is passionate about evidence-based medical care and the clinical research that drives it. He ensures his patients benefit from the most up-to-date and effective treatments.

When he's not working, the Tennessee native enjoys spending quality time with his family, exploring the outdoors through hiking and biking, and experimenting in the kitchen. A sports enthusiast, he also loves watching games and cheering for his favorite teams.

Orthopedics

Ryan Applone, DO

Dr. Ryan Applonie is a board-certified orthopedic surgeon and Fellow of the American Academy of Orthopedic Surgeons. He practices general orthopedics with a special focus on complex shoulder and elbow conditions, including arthroscopy, joint replacement, and fracture care.

Dr. Applonie completed his orthopedic surgery residency at Michigan State University- McLaren Macomb Hospital in Detroit, Michigan, in 2018, where he served as chief resident during his final year. He went on to complete a shoulder and elbow fellowship at the University of Connecticut in 2019.

After returning to Idaho in 2019, Dr. Applonie became an independent orthopedic surgeon and partner at Capital Surgical Associates in Boise. In addition to his private practice, he provides call coverage with the Adult Orthopedic Trauma Group at St. Luke’s Boise and Meridian. Dr. Applonie is excited to work with Valor Health to provide expert orthopedic care in Emmett alongside other independent orthopedic surgeons from the Treasure Valley.

An Idaho native raised in Idaho Falls, Dr. Applonie enjoys spending time skiing, hiking, camping, and fishing. He is an avid soccer fan and continues to play competitively.

Orthopedics

Amy Chan, PA

Amy Chan, PA-C is a board-certified physician assistant specializing in orthopedic surgery. She assists in both the clinic and operating room, providing comprehensive care for patients with acute and chronic orthopedic conditions, including joint preservation, arthroscopic procedures, and total joint arthroplasty.

Amy earned her Master of Physician Assistant Studies from Idaho State University in 2017 and holds a Bachelor of Science in Biological Sciences with a minor in Educational Studies from the University of California, Irvine. Prior to PA school, she trained and worked as an Emergency Medical Technician in Los Angeles. She is licensed to practice in Idaho and holds national certification as a physician assistant.

Before joining Capital Surgical Associates, Amy gained experience in family medicine, general surgery, and emergency care. She spent four years practicing family medicine in rural Oregon, two years in general surgery, and most recently focused on orthopedic surgery in the Treasure Valley. She brings extensive procedural expertise and a patient-centered approach, emphasizing open communication, close follow-up, and empathy.

Born and raised in Los Angeles, Amy has made the Boise area her home and enjoys the outdoors, community engagement, and supporting patients across Idaho with personalized, high-quality care. She is fluent in Cantonese, conversational in Mandarin, and intermediate in Spanish, allowing her to connect with a diverse patient population.

Tim Neuschwander, MD

Tim Neuschwander, MD, is a board-certified orthopedic surgeon specializing in sports medicine, arthroscopy, and joint replacement. He is a Fellow of the American Academy of Orthopedic Surgeons and a Diplomate of the American Board of Orthopedic Surgery.

Dr. Neuschwander earned his undergraduate degree from Dartmouth College and received his medical degree from the University of Southern California. He completed his orthopedic surgery residency at the University of California, San Diego, followed by an NIH research fellowship and a sports medicine fellowship at Santa Monica Orthopedic Group.

Prior to joining Valor Health, Dr. Neuschwander practiced in McCall for five years. His professional experience includes serving as assistant team physician for the LA Galaxy, volunteer team physician at the U.S. Olympic Training Center in Colorado Springs, and head orthopedic physician at Northern Michigan University.

Ray Jensen, DO

Dr. Ray Jensen is a board-certified, fellowship-trained orthopedic surgeon providing comprehensive musculoskeletal care for patients of all ages. He has specialized training in sports medicine and arthroscopic surgery, with particular expertise in shoulder care, joint preservation, and joint replacement procedures.

Dr. Jensen earned his Doctor of Osteopathic Medicine degree from A.T. Still University’s Kirksville College of Osteopathic Medicine. He completed his orthopedic surgery residency at Des Peres Hospital in St. Louis, followed by a sports medicine fellowship at the University of New Mexico.

Dr. Jensen is committed to delivering high-quality orthopedic care to patients across Idaho, helping individuals return to the activities they enjoy through personalized, evidence-based treatment.

Dermatology

Dr. Scott Thomas at Valor Health Hospital in Emmett, Idaho

Scott Thomas, MD

Richard Scott Thomas is a board-certified dermatologist who grew up in the Treasure Valley. After graduating from Nampa High School, he served a 2-year LDS mission in Madrid, Spain where he became fluent in Spanish.  His BS in Biology with a minor in Spanish was obtained from Brigham Young University.

After his undergraduate studies, he attended medical school at A.T. Still University/Kirksville College of Osteopathic Medicine in Kirksville, Missouri.  After a Transitional Internship at Good Samaritan Regional Medical Center in Corvallis, Oregon, Dr. Thomas completed his Dermatology Residency training at Western University of Health Sciences/Silver Falls Dermatology in Salem, Oregon.

At a young age, Dr. Thomas was taught by his parents to always put himself in the shoes of others.  This invaluable lesson gravitated him towards the field of dermatology as it allows him to treat physical dermatologic conditions while also treating the emotional well-being of patients.  Additionally, Dr. Thomas appreciates the diversity within the specialty, which allows him to treat conditions such as acne, warts, more chronic skin diseases such as psoriasis and rosacea, perform various cosmetic procedures, and treat potentially life-threatening skin cancer.

Treating skin cancer is of particular interest to Dr. Thomas as he received extensive Mohs micrographic surgery training during residency. He is a fellow of the American Society for Mohs Surgery.

Dr. Thomas is an avid sports fan, especially of BYU football and Utah Jazz basketball. His other hobbies include hiking, traveling, site seeing, running, and spending his free time with his wife and children.  He is extremely grateful to be back home in the Treasure Valley.

Pat Coleman, MD

Dr. Karl Undesser

Karl Undesser, MD

Karl P. Undesser, MD, is a board-certified cardiac electrophysiologist who has practiced medicine in the Boise area for over 18 years. He began his schooling in biology at the University of California, San Diego, and later received a PhD in pharmacology and doctor of medicine from the University of Texas Health Science Center. Over the next several years, Dr. Undesser completed post-graduate education and training in Texas, North Carolina and California, including an electrophysiology fellowship with the Division of Cardiology at University of California, San Diego. Dr. Undesser is board-certified in internal medicine, cardiovascular disease and electrophysiology and has extensive research experience with several publications.

Cardiology

Dr. Lyndon Box

Lyndon Box, MD

Dr. Lyndon Box is an experienced and board-certified interventional cardiologist. He graduated medical school at the University of Alabama at Birmingham School of Medicine, before completing fellowships in both cardiology and interventional cardiology at the University of Florida-Jacksonville. Prior to moving to Idaho, Dr. Box was part of the cardiology faculty at the University of Florida-Jacksonville, where he instructed training physicians and engaged in clinical research.

Dr. Box was born in Winfield, Alabama. His introduction to medicine was at the bedside of his father, who had open heart surgery when Dr. Box was just 8 years old and passed away from heart disease during Dr. Box’s freshman year of college. It was his father’s dream that Dr. Box be the first in his family to go to college, and it is not surprising that Dr. Box went into medicine.

Continually influenced by his childhood experiences, Dr. Box’s practice philosophy is based on the belief that the art of medicine consists of being able to utilize advances in science and technology without losing sight of the patient as an individual. He considers it his responsibility to make sure patients understand their conditions and feel comfortable with their care plan.

Dr. Box is married to a fellow physician and has three young children. As a family, they are actively engaged with their church community. Dr. Box and his wife both enjoy endurance sports and when he is not busy with work or family, you are most likely to find him out on the trails.

Robert E. Jackson, MD

Robert E. Jackson, MD treats all aspects of adult urology with a special focus on men’s health issues including vasectomies, male infertility, erectile dysfunction, incontinence, Peyronie’s disease, prostate cancer detection, kidney stones and benign prostatic hyperplasia (otherwise known and BPH or enlarged prostate).

Dr. Jackson is among the highest volume surgeons in the state for procedures including penile implants, artificial urinary sphincters, varicocele repairs, vasectomy reversals and surgical correction of Peyronie’s disease. He also has expertise in a wide spectrum of surgical treatment for BPH including RezūmTM, Urolift®, Greenlight Prostate Photovaprozation (PVP), Transurethral resection of the Prostate (TURP) and Holmium Laser Enucleation of the Prostate (HoLEP) allowing him to tailor his treatment plans to the patient’s individual needs and goals.

Throughout his career, Dr. Jackson has been at the forefront of urologic advancement in Idaho including performing the state’s first robot-assisted vasectomy reversal and leading the creation of the state’s first comprehensive Men’s Health Clinic and its first prostate cancer survivorship program.

Prior to starting the independent practice The Urology Clinic in 2023, Dr. Jackson served in multiple leadership positions at St. Luke’s Health System including Director of Men’s Health, Quality Officer for the Department of Urology and Medical Director of Boise/Meridian Urology.

He is a member of The American Urologic Association, The Sexual Medicine Society of North America and The American Assocation for Reproductive Medicine and is Board Certified in Urology.

Michael B. Taylor, MD

Michael B. Taylor, MD treats all aspects of adult urology with a focus in benign prostatic hyperplasia (otherwise known as BPH or enlarged prostate), kidney stones, men's health, urethral stricture disease, prostate cancer, and bladder cancer.

Dr. Taylor has pioneered several procedures in the state of Idaho.  He was the first urologist in the state to perform the following:

  • Holmium Laser Enucleation of the Prostate (HoLEP)
    • Dr. Taylor performed the first HoLEP in Idaho on July 31, 2022
    • In 24 months, he performed over 330 HoLEP procedures and led the training of local urologists in the technique. As of March 2025, he has conducted over 600 HoLEP procedures in Idaho with a >98% success rate
  • Urolift® (or prostatic urethral stenting)
  • Rezūm™ (prostate thermovaporization)
  • Buccal Graft Urethroplasty

Prior to starting the independent practice The Urology Clinic in 2023, Dr. Taylor served as Medical Director of the St. Luke's Meridian Medical Center Operating Room.
