Important Notice Regarding Behavioral Health Services Provider Lisa Isaksen


Dear Valued Patient,

We are writing to inform you that our Behavioral Health provider, Lisa Isaksen, NP-C, PMHNP-BC, will be departing from Valor Health to pursue new opportunities. Her final day with us will be June 1, 2026. Lisa has been an integral part of our care team, providing psychiatric services with a focus on diagnosing and treating mental health conditions across the lifespan. We are sincerely grateful for the knowledge and dedication she has shown to her patients and to our organization.


We are taking proactive steps to ensure your care continues without interruption. Our team is working closely with our primary care providers and collaborating with psychiatric providers in the community to help prevent any gaps in treatment or medication management. In addition, we are actively recruiting a qualified psychiatric provider and are currently interviewing candidates to fill this role. We will keep you informed as updates become available.


In the meantime, our care team is here to assist with medication refills, coordination of services, and any questions or concerns you may have during this transition. If you would like assistance with continuing your care, scheduling, or referrals, please do not hesitate to contact our office.
