Valor Health is proud to offer FREE sports physicals for GEM COUNTY STUDENT-ATHLETES

• Parents or guardians must sign sports physical forms, which are available here or on the day of the event.
• If you use corrective lenses or contacts, you must wear them.
• Please arrive 10 minutes early for check-in.
